Make a difference! Be a Chronic Disease Self-Management Program Peer Leader. Developed at Stanford University, this program is designed to empower those living with chronic conditions to better manage their own health and live a healthy, enjoyable life.
The training for the Chronic Disease Self-Management Program (CDSMP) will be held virtually on Zoom, from 1:00 to 3:30 p.m. on these dates:
